Formé clinic is a newly opened clinic in Prague, situated in an excellent location, right on the Wenceslas Square. We are the plastic surgery clinic, providing a wide range of plastic surgery treatments. The aim of our clinic is to be different to other private facilities, namely by a quality service and offering a wide scale of services to our clients. Experience with Forme clinic

I would like to recommend the Forme clinic. I was here two years ago and I decided to come back again. Dr Vasek is the man in the right place, funny, incredibly human and his experience and immense professionalism make him a great person. The staff was very nice and attentive. I felt safe at the clinic.

Breast augmentation with Dr Lhotsky

I'm satisfied with the result of my breast augmentation surgery. Dr Lhotsky did an excellent job and I am glad I chose him. He was interested and available to me the whole time before and after the procedure, answering all my questions.

I am extremely satisfied with the result after the otoplasty. The surgery was performed by Dr Bartoskova. During the consultation she explained the operation, she was very honest and polite. The operation went well and I no longer have protruding ears.

Liposuction by Dr Baran

I will gladly recommend the Forme Clinic. I felt very comfortable and safe there. The doctor and nurses provided great care. The liposuction was done under local anesthesia, but it didn't hurt at all. I was able to leave the clinic immediately after the procedure. The result is very good.
