Formé clinic is a newly opened clinic in Prague, situated in an excellent location, right on the Wenceslas Square. We are the plastic surgery clinic, providing a wide range of plastic surgery treatments. The aim of our clinic is to be different to other private facilities, namely by a quality service and offering a wide scale of services to our clients. Comfortable stay

Lovely staff made my stay very comfortable. The clinic is good located and they helped me with the airport transfer and also accommodation. After the surgery I stayed couple of days in close hotel and then I was free to go. I healed very well and it has been already six months and the result I brill. Would recommend to everyone

I had liposuction done by dr. Krizko. The surgery did not last too long and after few hours I could go home.  The result is very good. It takes little time to heal but I was totally fine after one month. The bruises were almost gone and so the swelling.

I experienced rhinoplasty with dr. Vasek. Must confirm that whole surgery process was organized pretty well and everyone was very nice and caring. My nose was little swollen after the surgery but in couple of weeks i felt good. The result is as i desired so i am happy

New breasts

It has been years to get courage to undergo breast augmentation. I thought i was gonna have those small breasts for rest of my life. My friend recommended Forme clinic where she had surgery herself. The best decision i made. Could not be more happy about my new size breasts and grateful to dr. Hyza for such a good job.

Love my new tummy

After giving birth to twins my tummy had very loose skin. I thought it will just go back after few months but even after one year it was still loose a lot. And however i did not plan another child i wanted to gain my flat tummy back. I am not really fan of plastic surgeries but this was the best i could do to love myself again.
